POLICE RECORD CHECK FORMS

All coaches, assistant coaches and managers are required to provide a Police Record Check to OMHA. Please see the following link for the document you will need to provide to the RCMP detachment in your jurisdiction:

You must present two forms of identification upon application. There will be a $5.00 fee for the initial processing, and a $10.00 fee if fingerprinting is required. Payment must be in cash.

Once you have received your police record check please email Lindsay Graw at lindsaygraw@shaw.ca to inform her that you have mailed the ORIGINAL copy to: OMHA, Box 1152, Okotoks, AB, T1S 1B2. If you would like to keep the original please contact Lindsay Graw at lindsaygraw@shaw.ca to make alternate arrangements.

The Okotoks Minor Hockey Association (OMHA) administers a minor hockey program for residents in Foothills County, including the Town of Okotoks. The current OMHA hockey program exists for players ages 5 years through 18 years, and consists of High Performance Level (AAA, AA), Community Level (A,B and C)

OMHA has a working partnership with 3 organizations to administer the AAA and AA levels of hockey - the OOAA (Okotoks Oilers Athletic Association), FMHA (Foothills Bisons Hockey Association) and the Foothills AAA Hockey Association.
